All associates regardless of medical plan participation and all spouses/partners covered by Advocate medical benefits can receive a free biometric health screening.  Screening includes:  height, weight, Body Mass Index/body fat percentage, blood pressure, and finger-stick to determine cholesterol and glucose levels.

 

How to Schedule a Screening

Advocate EPO or PPO Plan and Wellness Benefit Participants

    • Step 1

      • Go to advocatebenefits.com.
      • Select Access Healthe You.
      • Log in to your Healthe Rewards Page.  Follow the instructions on the login page the first time you log in.
    • Step 2

      Once logged in, complete the 2018 Health Risk Questionnaire (HRQ) Consent Form and Health Risk Questionnaire. 

      Note:  You must complete the HRQ before you can schedule a screening.

    • Step 3

      Click on the Schedule Screening icon to schedule and complete the screening consent form.

 

Advocate HMO Participants, Associates Newly Enrolled in Medical Benefits, Associates Not Participating in Medical Benefits

Our goal is to help you achieve your best health today and during retirement by providing education, support, resources and rewards. Healthe You is also a way to proactively manage our health care costs by helping participants manage and improve their health.
    • Who can participate in Healthe You?

      • Team members enrolled in Advocate Centered EPO or PPO Plan medical benefits and their covered spouse/partner can earn a Healthe Rewards Credit to lower their out-of-pocket medical expenses; can participate in cash raffles by earning Healthe Rewards Points; and have access to comprehensive resources and support programming to help manage their health.
      • Team members enrolled in Advocate Centered HMO medical benefits and their covered spouse/partner can participate in a free health screening; can earn Healthe Rewards Points for cash raffles; can participate in educational events and a smoking cessation program, and may take advantage of health and fitness-related discounts.
      • Team members not enrolled in medical benefits offered by Advocate can elect to participate in the wellness benefit and pay a $100 fee. This will allow access to participate in all support programming and the cash raffles.
      • Team members not enrolled in medical benefits offered by Advocate or who do not elect the wellness benefit can participate in a free health screening, educational events, smoking cessation program, and may take advantage of health and fitness-related discounts.
    • What incentives are offered and how do I earn them?

      Team members and their spouse/partner enrolled in the Advocate Centered EPO or the PPO medical plan can earn a Healthe Rewards Credit to help lower their out-of-pocket medical expenses:

      Advocate Centered EPO/DRP or PPO/DRP Plan Participants can earn up to:
      $800 team member
      $800 covered spouse/partner

      To earn your Healthe Rewards Credit, you must complete all program requirements. The first two steps are to complete the Health Risk Questionnaire and schedule and complete an Initial Healthe Measures Screening. Deadlines apply.
    • How do I avoid the tobacco surcharge?

      The tobacco surcharge is applied to team members participating in medical benefits and their covered spouse/partner who are tobacco users or who have not screened tobacco-free.

      If you and your covered spouse/partner have previously screened tobacco-free: Complete the Tobacco Affidavit after you complete the Health Risk Questionnaire on your Healthe Rewards Page. 

      If you have not previously screened tobacco-free or you have quit tobacco for six months: Call the Healthe You Solution Center at 1.800.937.5717, 8 am-8 pm, M-F, to schedule a screening to update your status.

      If you use tobacco but want to quit and avoid the surcharge: Call the Healthe You Solution Center at 1.800.937.5717, 8 am-8 pm, M-F, to enroll in the Healthe Breathe tobacco cessation program. Participants will be reimbursed for the tobacco surcharge at the end of their six-month program completion month (rolling calendar year). Participants must complete the Healthe Breathe Program each year if they have not fully quit tobacco to be eligible for the surcharge reimbursement.
